• PSA Birth Certificate
• 2x2 picture with white background 
• Certified True copy of Transcript of Records or Certification of Grades (online grades are not accepted unless Certified)
• Honorable Dismissal or Transfer Credential
• Not more than 3 units of failed courses/subjects
