What are the requirements for incoming college freshman applicants?

• Interactive Recommendation Form (click here to download)

• PSA Birth Certificate

• 2x2 picture with white background and name tag

• Certified true copy of Grade 11 Report Card (any quarterly grades available)

• Certification of not enrolled to any Colleges or Universities (if already graduated in previous  school year/s)
