What are the requirements for incoming Grade 11 applicants?

• Interactive Recommendation Form (download here)
• PSA Birth Certificate
• 2x2 photo with white background and name tag
• Certified true copy of the latest completed report card. 
  Example: If you're in Grade 10 and applying for Grade 11, submit your Grade 9 report card. If you've finished Grade 10, submit your Grade 10 report card.

* De La Salle Lipa does not accept applicants for Grade 11 second semester or Grade 12 transfer.
